The legal stuff.

Before all initial assessments, permission needs to be obtained from a Veterinary Surgeon to adhere to The Veterinary Surgeons Act (1966) and the Veterinary Surgery (Exemptions) Order (2015). Please request this through your Veterinarian or you can do this through Maddie prior to the first consultation. This is a legal requirement for any practitioner that will see your pet.

Veterinary referral certifies that your vet has given permission for MVP to perform veterinary physiotherapy on the stated animal. This protects each animal from potentially receiving treatment that may not benefit them if they have specific underlying health conditions. The owner will be required to complete an owner consent form form during the initial consultation which certifies that they give permission for MVP to work with their pet.

Payments will be accepted through cash, cheques, or bank transfer. If you wish to claim veterinary physiotherapy on your insurance please contact your insurance provider before any consultations. Maddie Veterinary Physiotherapy strives to provide an excellent service and respond to any issues promptly, and ask that in such an event, you contact Maddie at your earliest convenience.

Privacy Policy

This Privacy Policy describes and explains how and why any Personal Data or information you provide is used. All efforts are ensued to make sure your privacy is protected and used safely. If certain information is asked of you on this website, then you can be assured that it will only be used according to this privacy statement. This policy is effective from the 1st January 2022.

Data collected - Data you provide MVP Registration and payments If MVP is visiting your home for an appointment, you will need to provide your address. If you decide to pay online, you will need to provide payment (e.g. credit card) and billing information. Any personal data will be kept in a password protected file on a password protected computer. Other information collected include Geolocational data, IP addresses and data collected via cookies.

(a) Cookies and Tracking

The website and its partners may use various technologies to collect and store information when you use the services. This may include using cookies and similar tracking technologies on this website, such as pixels and web beacons to analyse trends, administer the website, track users’ movements around the website and gather demographic information about the user base as a whole. Users can control the use of cookies at the individual browser level.

(b) Third Party Access

Trusted third party affiliates are used in this website such include the Blog and data storage. These third parties may access, process or store your information to perform tasks only for the purposes that have been authorised. They provide at least the same level of protection as described in this Privacy Policy. We also may share with third parties aggregated or anonymised information that does not directly identify you. How is the data collected used? - to contact you and for payment Either via phone or email to discuss appointments or for updates on the business, offers and marketing emails (you can opt out of marketing emails).

Your address will be provided if MVP travels to your home or yard for an appointment so that we know where to meet. Payment and billing information will be used to take payments only. Data retention Personal information is retained for as long as needed or permitted in light of its purpose(s) for which it was obtained and consistent with applicable law. The criteria used to determine our retention period include:

  • All data is required to be retained for 7 years after the last treatment of your animal as specified by MVPs insurance and governing body: NAVP.
  • Whether there is a legal obligation to which we are subject (for example, certain laws require us to keep records of your transactions for a certain period of time before we can delete them.
  • Whether retention is advisable in light of my legal position (for example, in light of applicable statutes of limitations, litigation or regulatory investigations).

Your personal data is required to enable MVP to keep detailed records about your animal(s) and the treatments they require for my insurance company. Dispute resolution or filing a complaint If you have any complaints regarding the compliance with this Privacy Policy, please contact me first.

I will investigate and attempt to resolve complaints and disputes regarding use and disclosure of personal information in accordance with this Privacy Policy and in accordance with applicable law.

Changes to this privacy statement Revision of this Privacy Statement may occur from time to time to reflect changes to my business, the website or service, or applicable laws. The revised Privacy Statement will be effective as of the published effective date.
